Bernice Howard, 76, of Corbin, died Thursday, Feb. 7, 2013, at Baptist Health Corbin.
Survivors include a daughter, Linda Couch, of London.
Funeral services will be at 2 p.m. Monday at Bob Fork Holiness Church in Leslie County. Burial will follow at Cornett Cemetery in Rockhouse.
Visitation will be after 6 p.m. Saturday at the funeral home chapel, and from 2 p.m. Sunday until the funeral hour Monday at the church.
London Funeral Home is in charge of arrangements.
